Recovery Coordinator

As a Recovery Coordinator, you will be an integral part of a multidisciplinary team, delivering high-quality, person-centred care for individuals affected by drug and alcohol misuse. You will support individuals throughout every stage of their recovery journey, from initial triage and assessment through to structured treatment and sustained recovery.
Working collaboratively, you will develop and implement tailored recovery plans that are strengths-based, promote wellbeing, and reduce harm. You will facilitate access to a wide range of interventions, including psychosocial support and community resources, helping individuals build recovery capital and achieve positive, lasting outcomes for themselves, their families, and the wider community.
You will take ownership of a defined caseload, ensuring effective case management, continuous engagement, and coordinated care across partner agencies, always striving to improve outcomes and promote independence.
About You
You are a compassionate, organised, and proactive professional with experience supporting individuals affected by substance misuse in a health, social care, or community setting. You are confident in carrying out comprehensive assessments, developing recovery and risk management plans, and delivering evidence-based interventions tailored to individual needs.You excel at building positive, trusting relationships and are committed to empowering individuals throughout their recovery journey. What You’ll Do
- Work collaboratively with partner agencies and multidisciplinary teams to complete assessments, deliver key working sessions, and provide interventions that support progression from engagement through structured treatment to sustained recovery.
- Manage a defined caseload, developing, implementing, and regularly reviewing personalised recovery and risk management plans in partnership with individuals, colleagues, families, and significant others where appropriate.
- Facilitate access to appropriate treatment pathways, psychosocial interventions, community resources, and peer support, promoting social inclusion, harm reduction, and sustained recovery.
- Deliver evidence-based one-to-one and group interventions, ensuring interventions are responsive to individual needs and aligned with best practice.
- Maintain accurate, up-to-date records and contribute to case reviews, team meetings, and multi-agency discussions.
- Work in line with safeguarding procedures, clinical governance standards, and organisational policies at all times.
To Succeed in This Role, You’ll Need
- Knowledge of harm reduction approaches in relation to substance misuse and recovery-oriented models of care (practical experience is desirable).
- Understanding of, and ability to deliver, evidence-based interventions such as: Brief Interventions, Motivational Interviewing (MI), Psychosocial Interventions (PSIs), Cognitive Behavioural Therapy (CBT), ITEP (Intensive Treatment Effectiveness Programme)
- Experience in assessment, recovery planning, and developing effective risk management and safeguarding plans.
- Ability to manage a diverse caseload, prioritise workload, and work both independently and as part of a team.
- Strong communication and interpersonal skills, with the ability to engage and motivate individuals from a range of backgrounds.

Peripatetic Recovery Coordinator

We’re looking for a compassionate and motivated Recovery Coordinator to join our County Durham Drug and Alcohol Recovery Service, working within the Sustained Recovery Team and supporting the Recovery Academy Durham (RAD) programme.
The Recovery Academy Durham is a structured 12-week abstinence-based programme designed to support people in achieving and maintaining long-term recovery. Through evidence-based psychosocial interventions and activities centred around the Five Ways to Wellbeing, you’ll help people build confidence, resilience and the skills needed to sustain positive change.
As a Recovery Coordinator, you’ll work directly with people affected by drug and alcohol use, supporting them throughout every stage of their recovery journey. You’ll manage your own caseload, carrying out assessments, developing personalised recovery plans and delivering ongoing keywork interventions that empower individuals to achieve their goals.
In addition to delivering the RAD programme, you’ll provide flexible operational support across the Sustained Recovery Team. This includes covering staff absences, supporting caseloads across different locations, helping to reduce waiting times and ensuring continuity of care for the people we support.
Working collaboratively with colleagues, partner agencies, families and carers, you’ll help create a positive, inclusive and therapeutic environment where people feel empowered to make meaningful and lasting change.
What you’ll be doing
You’ll:
- Deliver the Recovery Academy Durham (RAD) pathways and interventions, ensuring programmes are engaging, evidence-based and person-centred.
- Deliver a comprehensive psychosocial programme alongside activities that promote the Five Ways to Wellbeing.
- Manage a caseload of service users, reviewing ongoing care plans and coordinating treatment throughout their recovery journey.
- Complete assessments, risk assessments and safeguarding processes.
- Build positive relationships with service users, families and partner agencies to support sustained recovery.
- Promote an inclusive, therapeutic environment where participation, engagement and feedback are encouraged.
- Provide operational support across County Durham services by covering absences, supporting caseloads and helping reduce waiting lists.
- Maintain accurate case records and contribute to high-quality service delivery.

Lead Practitioner

We’re looking for an experienced and motivated Lead Practitioner to join our County Durham Drug and Alcohol Service.
This is an exciting opportunity for someone with experience in substance misuse services who is ready to take the next step into a leadership role or build on their existing management experience. You’ll play a key part in supporting the day-to-day running of the service, leading a small team, maintaining high standards of care and ensuring the people we support receive safe, effective and person-centred interventions.
Working closely with the Service Manager, you’ll help drive service quality, mentor colleagues and contribute to the ongoing development of the service while continuing to hold a small caseload.
What You’ll Be Doing
As a Lead Practitioner, you’ll:
- Support the day-to-day operational management of the service and deputise for the Service Manager when required.
- Provide line management, supervision and coaching to a small team, helping colleagues develop and thrive.
- Deliver and oversee high-quality assessments, risk assessments and recovery planning.
- Maintain a small caseload, delivering evidence-based interventions to people accessing the service.
- Monitor service quality, analyse performance information and contribute towards achieving contractual targets and KPIs.
- Build strong relationships with internal teams, partner agencies and stakeholders across County Durham.
- Help shape and improve service delivery, ensuring services remain accessible and responsive to local need.

About You

- An NVQ Level 3 in Health & Social Care (or equivalent or above).
- Experience working within drug and alcohol services.
- Experience delivering evidence-based interventions and managing complex caseloads.
- Experience completing comprehensive assessments and risk assessments.
- Experience supporting or leading colleagues through supervision, coaching or line management.
- Strong communication skills with the confidence to present information and build relationships with stakeholders.

Recovery Coordinator

We’re looking for a caring, motivated and organised Recovery Coordinator to join our Hope Team.
You’ll manage your own caseload, working closely with individuals to understand their goals, develop personalised recovery plans and provide ongoing support throughout their recovery journey. No two days are the same—you could be completing assessments, delivering one-to-one interventions, facilitating recovery groups, working with partner agencies or supporting people to access wider community services.
This is a rewarding opportunity to make a genuine difference to people’s lives while becoming part of a supportive and experienced multidisciplinary team.
The role is primarily Monday to Friday, 9:00am-5:00pm, with one early finish each week. You’ll also take part in a rota covering approximately one late evening each month and one Saturday morning every 12 weeks to help ensure the service remains accessible to those who need it.
What You’ll Be Doing
- Managing a caseload of people accessing drug and alcohol treatment and recovery support.
- Completing assessments, recovery planning and ongoing reviews.
- Delivering evidence-based one-to-one and group interventions.
- Working in partnership with health, social care and community organisations.

- Undertaking safeguarding, risk assessments and case management.

Harm Reduction Outreach Worker

We’re looking for a proactive and passionate Harm Reduction Outreach Worker to join our County Durham Drug and Alcohol Recovery Service (CDDARS). This is an exciting opportunity to make a real difference by reducing drug-related harm and improving health outcomes for individuals and communities across County Durham.
As an integral member of our Harm Reduction team, you’ll deliver evidence-based interventions that help keep people safe, improve access to treatment and support recovery. Working across a variety of community settings, you’ll build strong relationships with partner agencies and promote harm reduction initiatives including Needle Syringe Programmes (NSP), Naloxone provision, Blood Borne Virus (BBV) testing and Hepatitis C awareness and treatment.
This is a varied outreach role where no two days are the same. You’ll have the autonomy to plan your own workload while working closely with colleagues across the wider service to ensure people receive the support they need, when they need it.
About the service
County Durham Drug and Alcohol Recovery Service (CDDARS) provides specialist support to people affected by drug and alcohol use across County Durham. Our dedicated multidisciplinary team delivers a range of interventions designed to reduce harm, improve wellbeing and support lasting recovery.
The Harm Reduction team plays a vital role within the service, working with individuals, communities and partner organisations to increase awareness, improve access to life-saving interventions and embed harm reduction into wider health and social care provision.
About you
You’ll be passionate about supporting people to improve their health and wellbeing and thrive in a role where you can build relationships and make a visible difference within local communities.
You’ll bring experience of working within substance misuse services or a related field, alongside an understanding of safeguarding, harm reduction principles and evidence-based interventions. You’ll be confident working independently in community settings while also being an effective team player.

- Experience delivering interventions on a one-to-one and group basis.
